Abstract | 「要旨」Postextrasystolic Potentiation (PESP) はviableな心筋や心収縮予備能を把握する鋭敏な指標である. われわれは心電図同期心プールシンチグラフィを用いPESPの評価を試みた. 経静脈的に右室心尖部に電極カテーテルを挿入し, cardiac stimulatorを用いて心室性三段脈を作成する一方, 心プールシンチグラフィはリストモードで12分間データ採取した. これから編集された期外収縮後第1拍とコントロール心拍を比較検討した. 期外収縮後第1拍はコントロール心拍に比べ, 拡張末期容積は増加, 収縮末期容積は減少, 駆出率および最大駆出速度は増加した. コントロール心拍でみられた壁運動異常が期外収縮後第1拍では改善する例が存在した. 本法は心筋のviabilityや心収縮予備能を評価する有用な方法と考えられる. |

Abstract | [Summary] Postextrasystolic potentiation (PESP) is an excellent approach to predict myocardial viability and inotropic reserve of the ventricle. We evaluated PESP by means of gated radionuclide angiocardiography. The electrode catheter was positioned in the apex of the right ventricle, and ventricular trigeminy was generated by programmed cardiac stimulator. Radionuclide ventriculography with Tc-99m HSA was performed in a modified left anterior oblique projection, and list mode data were acquired for l2 minutes. From these data three separate clusters of cardiac cycles were identified. Compared with the functions derived from control beat, in these from the postextra beat, end-diastolic volume was increased but end-systolic volume was decreased, and both ejection fraction and peak ejection rate were increased. Abnormal wall motion found in the some cases of ischemic heart disease was improved in postextra beat. These results indicate an usefulness of evaluation of myocardial viability and of ventricular inotropic reserve by using radionuclide method. |
