Abstract | 「要旨」熱障害赤血球による選択的脾シンチグラフィの適応と意義を検討するため, Tc-99mコロイドシンチグラフィを同時に施行できた19症例を対象に両者の所見を比較検討した. 脾内限局性病変疑いの患者 (n=8) では, コロイドシンチグラフィで脾の取り込みがすくない場合や, 欠損の有無が不明瞭な症例で, その形態的評価が可能であった. 白血病の2症例では, 細網内皮系にコロイドの取り込みが著しく低下しているにもかかわらず, 脾シンチグラフィで明瞭な取り込みがあり, functional aspleniaと診断された. 脾組織の有無を検討した症例 (n=5) では, 全例でその有無が判定できた. 熱障害赤血球による脾シンチグラフィは, 手技も比較的簡便であり, 脾組織の確認が必要な場合は, 絶対的適応となるが, コロイドシンチグラフィで所見が不明確な場合も有効な検査法である. |

Abstract | [Summary] To evaluate the indication of splenic scintigraphy using Tc-99m-heat-denatured red blood cells (DRBC), 19 patients were studied retrospectively, and compared with the findings of Tc-99m-tin-colloid (TC) scintigraphy. In 8 patients suspected of having splenic defects on TC images, the DRBC scintigraphies provided better morphological informations. Two patients with leukemia were diagnosed to have functional asplenia, because TC images showed markedly reduced splenic radioactivity, while DRBC images demonstrated splenomegaly with normal accumulation. In all 5 patients who showed no spleen on TC images, the presence or absence of the spleen was easily determined. The technique of labeling with DRBC was simple. The identification of splenic tissue is considered to be an absolute indication of DRBC scintigraphy; however, when TC scintigraphy shows equivocal splenic findings, DRBC scintigraphy can be a valuable method. |
