Abstract | 「要旨」確定診断がなされた83症例 (肝内SOL 27例含む) を対象として, 核医学医師4人の判読により, 肝内SOL検出におけるSPECTとconventional imagingの比較検討を, ROC解析により行った結果, 以下のごとき結論に達した. 1) 肝内SOLの検出には, 両検査の併用が最も良好な結果を示したが, 単独の比較では, SPECTがより良好な結果を示した. 2) 深部小病変や肝RI摂取低下例での病変の検出には, SPECTの方が明瞭にSOLの存在を指摘できた. 逆に表在性小病変の検出には, conventional imagingの方が明瞭にSOLの存在を指摘できた. 以上, 核医学検査による肝内SOL検出向上には, SPECT conventional magingを単独で施行することなく, conventional imagingに引き続いてSPECTを実施することの有用性が示唆された. |

Abstract | [Summary] Eighty-three patients including 27 patients with focal hepatic disease were examined to evaluate the clinical efficacy of single photon emission computed tomography (SPECT) for the detection of space occupying lesions (SOL) in the liver, in comparison with that of conventional imaging. SPECT was subsequently performed using a dual-head rotating gamma camera after conventional imaging with LFOV gamma camera. The presence or absence of SOL in the liver was evaluated by four nuclear medicine physicians having more than five years experience, first in the SPECT images alone, secondaly in the conventional images laone, and lastly in the both images. According to the ROC analysis, the clinical efficacy of SPECT was much better than that of conventional imaging for the detection of SOL in the liver, although the combination of conventional imaging and SPECT was the best. In six out of 27 patients with focal hepatic desease, the SOL in the liver was not detected by conventional imaging, but clearly detected by SPECT. Four of these six cases were the small SOL at a deep portion of the liver, and the remained two cases were associated with the multiple small SOL in the low radionuclide hepatic activity. In two cases, however, the small SOL at a superficial surface of the liver, easily detected by conventional imaging, was not detected by SPECT alone. In the present study, it is suggested that the SPECT may be necessary as an adjunct to conventional imaging in order to improve the clinical efficacy for the detection of SOL in the liver. |
