Abstract「要旨」従来の心電図同期心プールシンチグラフィでは不可能であった心室性期外収縮の左右心室収縮能および心収縮様式をfunctional imageを作製して検討した. 対象は64歳女性の洞機能不全症候群 (症例1) と50歳女性の代償性心室性二段脈 (症例2) である. 症例1においては右房および右室ペーシングにより人工的に代償性心室性二段脈および三段脈を作製し, 左前斜位平衝時リストモード心電図同期法により期外収縮を含むすべての心拍をコンピュータに入力した. そしてRR時間の違いから洞調律と心室性期外収縮を区別して, 洞調律と期外収縮の心プールシンチ像とそのfunctional imageをおのおの作製した. 洞調律に比して期外収縮の左右心室global EFは低値を示し, EF imageでも左右心室の壁運動低下が示された. また期外収縮のphase imageではその発生源を推定することができた. リストモード法を用いた本法は期外収縮そのものの解析に有用な方法であると思われた.

Abstract[Summary] The purpose of this report is to evaluate the global and regional ventricular contraction and ventricular contraction pattern of ventricular premature beat (VPB) by cardiac blood pool scintigraphy. All beats containing artificial or spontaneous VPB were recorded by ECG-gated list mode data aquisition in a patient (case 1) with artificial ventricular bigeminy and trigeminy which were programed by right atrial pacemaker and right ventricular pacemaker that was located in right ventricular inflow tract on chest X-ray films and in a patient (case 2) with sponteous ventricular bigeminy. Sinus beats (SB) and VPB were differentiated by RR interval histogram which showed two peaks in bigeminy, and three peaks in trigeminy. Short RR interval, middle RR interval and long RR interval corresponded to SB, post-VPB SB and VPB, respectively. Left and right ventricular global EF and regional wall motion estimated by EF images were reduced in VPB than in SB. There were no differences of phase angles between left and right ventricule on phase images of artificial atrial beats in case 1 and spontaneous SB in case 2, however left ventricular phase delays were observed on phase images of artificial VPB in case 1 and spontaneous VPB in case 2. In case 1, the site of earliest phase angle located at right ventricular inflow tract on the phase image of artificial VPB. In case 2, the site of earliest phase angle was observed at right ventricular outflow tract where was predicted as the focus of spontaneous VPB by electrocardiogram. This method using ECG-gated list mode data aquisition was avairable to evaluate the global and regional ventricular contraction and ventricular contraction pattern of VPB.
