Abstract | 「要旨」われわれの開発した超音波装置つきシングルプローブ装置では, 左室駆出率 (LVEF) を初回通過 (FP) 法, 平衡時, beat by beat (B-B) 法, 心電図同期加算 (MG) 法の3方法によって求めることができる. 40名の患者で本装置による上記3方法を用いて求めたLVEFとシンチカメラで求めたそれとを比較した. FP法, B-B法, MG法で求めたLVEFとシンチカメラで求めたそれとの相関係数 (r) は, 0.938, 0.932, 0.930となり, いずれも高い相関が認められた. それぞれの平均値の間に有意な差は認められなかった. 15症例でFP法を2回連続して実施し, 求めたLVEFを比較したところ, r=0.953という高い相関が認められた. 12症例で同一検査がB-B法を5回反復施行し, 得られたLVEFを比較した結果, 再現性は良好であった. |

Abstract | [Summary] We developed a computerized cardiac single probe system combined with B-mode ultrasound instrument. With this system left ventricular ejection fraction (LVEF) can be estimated by the following three modes; first pass (FP) mode, beat-by-beat (B-B) mode, and ECG multigated (MG) mode. The purpose of this paper is to describe the accuracy and reproducibility of the LVEFs estimated by this system. In 40 patients with various heart disease, the LVEFs obtained with this system by above mentioned 3 modes were compared with those of obtained by gamma camera. There were good correlations between the LVEFs obtained by 3 modes of this system and those of obtained by gamma camera. The correlation coefficients (r) between the LVEFs obtained by FP mode, B-B mode and MG mode, and those of obtained by gamma camera were 0.938, 0.932, and 0.930, respectively. The correlation coefficient between the LVEFs obtained by MG mode and those of obtained by B-B mode was 0.970. In 15 patients FP mode studies were performed twice and LVEFs obtained in the initial and repeat studies were compared. There was good agreement between initial and repeat FP mode estimation (r=0.953) . In 12 patients positioning of the probe over the left ventricular and background regions were repeated 5 times and LVEFs estimated by B-B mode. There was no significant standard deviation compared to the mean LVEFs in each patient. In conclusion, our results suggest that the computerized probe system is a sensitive, rapid and relatively inexpensive portable method of screening for cardiac dysfunction with a yield similar to that from the more costly gamma camera system derived radionuclide angiocardiography. |
