Abstract | 「要旨」Segmental biliary obstruction (SBO) 11例と, 肝胆道疾患陰性29例の検討結果から, 次のごとき結論を得た. 1) SBOの肝胆道シンチグラム像は, 閉塞部および閉塞部より末梢の肝内胆管がまったく描出されない完全閉塞と, 閉塞部および閉塞部より末梢の肝内胆管の描出遅延と排出遅延の不完全閉塞とに大別された. 2) 一次分枝より末梢での完全閉塞の場合は, SBOの存在を指摘することは困難であったが, 一次分枝の完全閉塞および不完全閉塞では, その特徴的シンチグラム所見からSBOの存在を容易に指摘することが可能であった. 以上, 肝胆道シンチグラフィは, SBOの臨床的評価法として有効と思われた. |

Abstract | [Summary] Eleven patients with segmental biliary obstruction (ten patients with intrahepatic stone and one patient with cholangiocellular carcinoma) and twenty-nine patients without hepatobiliary disease were examined to evaluate the usefulness of hepatobiliary scintigraphy in segmental biliary obstruction. Nine out of eleven patients (82%) with segmental biliary obstruction showed the characteristic segmental abnormalities on the hepatobiliary scintigram, and the determination of segmental obstruction was easily made. In these nine patients, seven showed the incomplete obstruction of the delayed filling and prolonged retention at the involved obstructive intrahepatic duct. And the other two patients showed the complete obstruction of the absence of radioactivity at the involved primary branch of the intrahepatic duct. In the remained two patients only (18%) , no segmental abnormalities could be pointed out on the hepatobiliary scintigram, although the involved obstructive intrahepatic duct was not visualized. It is suggested that the hepatobiliary scintigraphy may be accepted as one of the useful non-invasive procedures for the detection of the segmental biliary obstruction. |
