Abstract | 「要旨」First pass法を用いた心RIアンジオグラフィを正常者, 虚血性心疾患患者, 右室圧負荷のある患者, 計50名に施行し, 安静時右室駆出率 (RVEF) , 容積曲線の一次微分dV/dtを収縮期および拡張期で算出し, 心臓カテーテルのデータと比較した. RVEFの正常群の値は50.8±8.7%であった. RVEFは右室圧負荷群と下壁心筋梗塞群で有意に低値 (それぞれ34.5±14.3% (mean±SD) , 41.3±6.2%) を示し, 右室機能低下が示された. さらにRVEFと収縮期最大dV/dtは右室の前負荷である右室拡張末期圧と相関を認めず, 後負荷である平均肺動脈圧, 肺血管抵抗等と有意の負の相関を示した (r=-0.46, p<0.01, r=-0.56, P<0.001) . 本法は右室機能を非侵襲的に評価する有用な検査法であると考えられた. |

Abstract | [Summary] We analyzed right ventricular function using first-pass radionuclide angiocardiography (FP-RNA) in 11 normal subjects, 28 patients with coronary artery disease (CAD) and 11 patients with right ventricular pressure overload. In normal subjects, right ventricular ejection fraction (RVEF) averaged 50.8+-8.7% (m+-SD) . RVEF in patients was significantly decreased than that in normal subjects. Several parameters (RVEF, maximal systolic and diastolic dV/dt) obtained by FP-RNA were compared with hemodynamic parameters obtained by cardiac catheterization. RVEF and maximal systolic dV/dt correlated with the mean pulmonary artery pressure and pulmonary vascular resistance (r=-0.46, r=-0.56) . While RVEF was less dependent on preload, highly dependent on afterload. In conclusion, we considered that the FP-RNA was a useful method to evaluate right ventricular function in CAD and other heart disease. |
