Abstract「要旨」 56例の急性心筋梗塞患者において, 99mTc-HMDPと99mTc-PYPによる心筋シンチグラムを比較検討した, 99mTc-HMDPの本症診断におけるsensitivityは90.3%であり, 99mTc-PYP (91.6%) のそれとは有意差を認めなかった. シンチグラム上での心筋への集積程度を評価するために算出したaverage graded intensityは, 両者間に有意差はなく, (99mTc-HMDP 2.78±1.45, 99mTc-PYP 3.07±0.95) ともに, global LVEF (r=-0.68) , 201Tl defect score (r=0.61) と良好な相関を示した. 99mTc-HMDP陽性例のうち16例について反復検査を施行したところ, うち3例では陽性所見が持続し, その予後は不良であった. 99mTc-リン酸化合物による反復検査は, 急性心筋梗塞患者の予後を判定する上で, 有用な検査法である.

Abstract[Summary] 99mTc-HMDP was compared to 99mTc-PYP in 56 patients with proven acute myocardial infarction (AMI) . No significant difference of sensitivity (HMDP 90.3%, PYP 91.6%) and average graded intensity (HMDP 2.78+-1.45, PYP 3.07+-0.95) between both radiopharmaceuticals was observed. Average graded intensity showed good correlation with both global LVEF (r=-0.68) and 201TI defect score (r=0.61) . 99mTc-HMDP was considered to be an ideal radiopharmaceutical like 99mTc-PYP, not only to diagnose AMI but also to estimate LV function in patients with AML 16 patients with positive 99mTc-HMDP scintigram were performed serial myocardial scintigraphy. Three out of these sixteen patients were still positive at 10 days (average) after onset. All of these patients went on unfortunate clinical course. This suggests that serial scintigraphy is available for assessment of expectant clinical course in patients with AMI.
