Abstract | 「要旨」正常肝および各種肝疾患86例に対する67Ga-citrate集積の経時的変化を比較し, 肝細胞癌診断における有用性を検討した. 測定方法は, 67Ga-citrate 3mCi静注後, 6, 24, 30, 54, 78時間ごとに, 前面, 後面像のcount収集を行い, その合計から 1 pixelあたりの平均値を算出した. 次に, 6時間値を100%として各時間値を補正しtime relation curveを求め, そのcurve patternを3型 (Type I減少型, Type II平坦減少型, Type III増加減少型) に大別した. Type I・IIは, 正常肝37例, 各種肝疾患32例に認められた. 一方, Type IIIは肝細胞癌14例, 正常肝3例の計17例であった. Type IIIをpositive, Type I・IIをnegativeとすると, 肝細胞癌に対する診断率はsensitivity 73.7%, specificity 92.5%, positive predictive value 82.4%, negative predictive value 88.1%であり, 質的診断を目的とした場合, 本検査法の意義は高いと考えられる. |

Abstract | [Summary] The use of 67Ga-citrate in diagnosis of hepatoma until recently was for determining the location and shape of the lesion. However, since 67Ga-citrate lacked specificity in regard to hepatoma, little expectation was held for 67Ga-citrate's usefulness in qualitative diagnosis. This study reports the results of our investigation to determine the usefulness of 67Ga-citrate for qualitative diagnosis by measuring the quantitative time-lapse changes in 67Ga-citrate accumulation in both normal subjects and subjects with diseased livers. The time-lapse changes in 67Ga-citrate accumulation were compared for 86 cases of normal (40) and diseased livers (46) by liver scintigraphy performed 6, 24, 30, 54, and 78 hours after intravenous injection of 3mCi 67Ga-citrate. Setting the 6-hr average value for 1 pixel at 100, the remaining hour values were revised accordingly and a time relation curve was established. Three curve patterns were predominant : Type I-decreasing, Type II-flatly decreasing, and Type III-increasing, then decreasing. Types I and II were recognized in 37 normal cases and in 32 cases of liver disease. Type III was seen only in hepatoma cases (14) and normal cases (3) , thus showing a definite correlation between hepatoma and the Type III curve pattern. Assuming Types I and II to be negative and Type III to be positive, a decision matrix was made for hepatoma. Looking at the diagnostic rate, sensitivity was found to be 73.7%, specificity 92.5%, positive predictive value 82.4%, and negative predictive value 88.1%. This examination method is thought to have great significance for qualitative diagnosis of hepatoma. |
