Abstract | 「要旨」Immunoradiometric assay法による血清サイログロブリン (Tg) の測定をH-TG kit (CIS-Sorin社) を用いて, 基礎的検討を行うとともに, バセドウ病の抗甲状腺剤による治療時のTg濃度について検討した. 健常者35例の血清Tg値は8.1±6.9ng/ml (mean±SD) であった. 甲状腺疾患では20例の未治療バセドウ病では274.5±158.3ng/ml, 急性期の亜急性甲状腺炎では, 250.6±187.1ng/mlと, 甲状腺機能亢進状態では, いずれも正常に比し有意の (p<0.01およびp<0.05) 高値を示した. 抗甲状腺剤投与により甲状腺機能が正常化しているバセドウ病を, 10例ずつ寛解群 (I) , 抗甲状腺剤維持量投与中で, 抗甲状腺剤治療期間2年未満群 (II) , 2〜3年群 (III) および4〜8年群 (IV) の4群に分けた. I群のTg値は25.9±16.2ng/mlであり, 他のII〜IV群の, それぞれ, 85.8±49ng/ml, 10.1±59.1ng/mlおよび142.8±94.9ng/mlに比し, 有意に低値であった. 本法は簡便かつ信頼性があり, 特にバセドウ病の抗甲状腺剤治療において, 寛解時期の判定のよい指標になるものと思われた. |

Abstract | [Summary] Serum thyroglobulin (Tg) concentration was measured by immunoradiometric assay with HTG kit (CIS-Sorin) . The mean Tg value was 8.1+-6.9ng/ml (+-SD) in 35 normal subjects. It was elevated in 20 patients with untreated Basedow's disease and 10 patients with hyperthyroidism due to subacute thyroiditis, the values being 274.5+-158.3 and 250.6+-187.1ng/ml, respectively. The 40 patients with Basedow's disease who were treated with antithyroidal drugs were subdivided into four groups : remission group (I, n=10) , antithyroidal medication for less than 2 years (II, n=10) , for 2-3 years (III, n=10) and for 4-8 years (IV, n=10) . The patients in group II-IV had been treated with maintenance doses of MMI (5mg) or PTU (50mg per day) . The value of Tg in group I (25.9+-16.2ng/ml) was significantly lower than those of group II, III and IV (85.8+-49ng/ml, 105.1+-59.1ng/ml and 142.8+-94.9ng/ml, respectively) . The level of serum thyroid hormone was almost the same in each group. All subjects studied had no circulating Tg autoantibodies as measured by anti-Tg radioimmunoassay kit (CIS-Sorin) . It is concluded that this method was simple, accurate and useful for routine examination, and that the evaluation of serum level of Tg was useful for decision of time of discontinuing antithyroidal drugs. |
