Abstract「要旨」 運動負荷201Tl SPECTを心筋梗塞 (MI) 後1〜3か月の患者72例 (一枝病変 : 34例, 多枝病変 : 38例) に施行し, MI患者が一枝病変か多枝病変かを推定し, その有用性と限界について検討した. 結果 : (1) 冠動脈病変の有病正診率は100%であった. (2) 一枝病変正診率は85%多枝病変正診率は71%であり, SPECTで多枝病変とした84 (27/32) が真の多枝病変であった. (3) 多枝病変患者を前壁梗塞群 (A群 : 11例) , 後下壁梗塞群 (P-I群 : 20例) およびA+P-I群3例の3群に分類した時, 梗塞部位の検出率は3群ともに100%であったのに対し, A群における後下壁病変の検出は36%, P-I群における前壁病変の検出は75%であった. 以上よりSPECTは心筋梗塞患者の多枝病変の検出を高い精度で可能とし, 特に後下壁梗塞に伴うLAD病変の検出には有益な情報をもたらすが, 前壁梗塞に伴うLCXまたはRCA病変の検出には限界があると考えられた.

Abstract[Summary] The purpose of this study is to evaluate clinical usefulness and limitations of stress 201Tl myocardial single photon emission computed tomography (SPECT) for detection of multivessel disease in patients with previous myocardial infarction. 72 patients underwent stress SPECT by symptomlimited graded bicycle ergometer exercise using dual-headed rotating gamma camera (TOSHIBA GCA 70A) equipped with computer system (GMS 90) . Transaxial, sagittal and coronal tomographic images were examined qualitatively. The results were as follows. 1) Sensitivity for detection of coronary artery disease was 100%, and sensitivity & specificity for detection of individual coronary arterial lesions by segmental analysis of SPECT images were fairly high (sensitivity ; LAD 88%, LCX 66% RCA 92%, specificity : LAD 93%, LCX 92%, RCA 82%) . 2) Sensitivity in detecting both single vessel disease (85%) and multivessel disease (71%) were fairly high. 3) In patients with a previous anterior infarction, concomitant RCA or LCX lesions were detected in only 4 of 11 cases (36%) , whereas in those with previous posterio-inferior infarction, the sensitivity for LAD disease was 75% (15/20) . Thus, stress SPECT is a useful noninvasive technique for the documentation of the number of affedted vessles in previous myocardial infarction, especially in postero-inferior infarction. However, SPECT proved to be a poor predictor of multivessel disease in patients with previous anterior infarction.
