Abstract | 「要旨」 Tl-201を用いた負荷心筋スキャンにおける定量的評価の有用性を検討するために, 負荷直後像でのTl-201の空間的分布 (E-R) と, 局所のwash out ratio (W-R) をcircumferential profile解析によって評価した. 局所wash outは心筋内6°ごと60点で計算したが, 負荷直後像と3時間後像との位置の対応を正確にするために, 心尖部が180°となるようにsampleさせた. 正常者8人からE-R, W-Rの診断基準を作成し, これ以下の部分をスコアとして計算した. 視覚的評価法はsensitivity (Sn) 58%, specificity (SP) 87%であった. 定量的評価においては, E-R単独ではSn 77%, Sp 73%, W-R単独ではSn 81%, SP 100%であり, E-R, W-Rの組み合わせでは, Sn 96%, Sp 73%となった. 心筋梗塞のある群では, 視覚評価のSnは85%, E-Rが100%, W-Rが69%であった. 心筋梗塞のない群では視覚評価のSnは31%, E-Rが54%, W-Rが, 92%であった. 局所wash outの異常は心筋梗塞を伴わない虚血巣の検出に有用と思われる. |

Abstract | [Summary] To investigate the clinical usefullness of quantitative evaluation of circumferential profiles of thallium-201 myocardial early distribution (E-R) and wash out ratio (W-R) , original computer program was developed. The method, applied here, uses bilinear interpolative back ground subtraction and circumferential profiles with automated selection of left ventriclar center and apex point to quantitate the relative activity in the myocardium, In addition, circumferential profiles of wash out ratio are calculated as percent wash out from the early and delayed (3 hours) circumferential profiles. Normal limites of E-R and W-R were obtained from 8 normal patients. Defect score and abnormal region corresponding with coronary arteries were evaluated by comparing with this normal limits. Three groups of patients were studied : 15 normal patients, 13 IHD with myocardial infarction (OMI) , 13 IHD without OMI (AP) . In the visual assessment, sensitivity (Sn) was 58%, specificity (Sp) was 87%. In the quantitative interpretation with BR alone, Sn was 77%, Sp was 73%, with W-R alone Sn was 81%, Sp was 100%. In the comparison of sensitivity between OMI and AP, visual and E-R showed high sensitivity for the detection of OMI, on the other hand W-R was most sensitive for the detection of IHD without OMI. |
