Abstract「要旨」 フーリエ解析は左室壁局所運動異常 (WMA) を検出する有用な方法である. WMAは選択的左室造影により評価されたakinesisまたはdyskinesis群 (Group I) , 局所的hypokinesis群 (Group II) , WMAを有しない群 (Group III) に分類した. Phase imageと超音波心臓断層法 (UCT) が45例に施行され, Group I, II, IIIはおのおの21人, 10人, 14人であった. RAO phase imageはapical, anterolateral, inferiorのpixelの位相平均の差が15度以上の時, WMAを有すると評価した. 正診率はGroup I, IIでそれぞれ76%, 20%でGroup IIIは79%であった. LAO phase imageは左室内pixelの位相の標準偏差が10度以上の時, WMAを有すると評価した. 正診率はGroup I, IIでそれぞれ90%, 80%でGroup IIIは93%であった. UCTの正診率はGroup I, IIでそれぞれ62%, 20%でGroup IIIは79%であった. 以上より, UCTに比してRAO phase imageはWMAの部位診断にやや優れ, LAO phase imageはWMAの存在の有無の診断に優れていると考えられた.

Abstract[Summary] Fourier analysis is a useful method to detect regional left ventricular (LV) wall motion abnormality (WMA) . WMA was evaluated by LV contrast angiography and defined as follows ; akinesis or dyskinesis (Group I) , localized hypokinesis (Group II) , and no WMA (Group III) . Phase image and ultrasonic cardiotomography (UCT) were performed in 45 patients comprised of 21 in Group I, 10 in Group II, and 14 in Group III. We evaluated WMA as being present when there was a difference of more than 15 degrees in the mean of pixel phase values of each apical, anterolateral and inferior segments in RAO phase image. The sensitivity was 76% and 20% in Group I and II, respectively, and the specificity was 79% in Group III. In LAO phase image, when the standard deviation of pixel phase values within LV was more than 10 degrees, its image was evaluated as WMA. The sensitivity was 90% and 80% in Group I and II, respectively, and the specificity was 93% in Group III. In UCT the sensitivity was 62% and 20% in Group I and II, respectively, and the specificity was 79% in Group III. In conclusion, RAO phase image was found to be slightly more useful for defining the site of WMA than UCT, while LAO phase image proved to be more beneficial for detecting the presence of WMA.
