Abstract「要旨」脚ブロック, WPW症候群, 右室ペースメーカなどの心興奮伝導異常例のRIマルチゲート心プール像に位相解析法を試み, その時間的な収縮異常の観点より, これらの症例における心室収縮様相の特徴, 左室機能との関連, さらに左右心室間の協調性の良否を正常例と対比検討した. また一部のペースメーカ例ではペーシングレート変換による心室収縮様式の変化をも検討した. その結果, 1) 位相分布ヒストグラムの平均値と心拍数との間に高い正の相関を正常伝導例のみならず, 心興奮伝導異常例においても認めた. 2) mean phase differences (LV-RV) により, これらを左室収縮が先行する群 (CRBBB, WPW A type) と右室収縮先行の群 (CLBBB, WPW B type, right ventricular Pacemaker) の2群に明瞭に大別できた. したがって, 本法は心興奮伝導異常を示す疾患の臨床評価に有用と考える.

Abstract[Summary] Phase analysis of biventricular radionuclide gated studies revealed markedly delayed phase in the affected ventricle of patients with bundle branch block, RV pacemaker and WPW syndrome. So that, we could classify these cases into 2 groups against normal conduction cases, judging from the point of the mean phase differences (LV-RV) , calculated together with the mean value and standard deviation (SD) of the phase distribution in each ventricle. First group were cases of LBBB, type B of WPW and RV pacemaker, which indicated an abnormal LV contraction sequence and LV delay. Second group were cases of RBBB and type A of WPW, which were characterized by an abnormal RV contraction sequence and RV delay. Also, our data suggested that the mean value was correlated with heart rate, but, the SD within LV wasn't correlated with left ventricula ejection fraction in these cases as well as normal conduction cases.
