Abstract「要旨」RIマルチゲートスキャン (以後RI法) による位相解析が左室収縮の協調性を評価し得ることに着目し, RI法を用いて心筋梗塞後心室瘤の定量的評価とその臨床的有用性について検討した. その結果, (1) シネアンギオグラムによるコントラスト法およびRI法で求めた左室駆出率の間には, 推計学的に有意な相関が得られた (r=0.62, p<0.02). (2) コントラスト法から求めた梗塞周径比 (ΔL) とRI法から求めた位相の標準偏差 (SD) との間には, 良好な相関が得られた (r=0.83, p<0.001). ΔL 35%はSD21度に相当する値と考えられ, 心室瘤切除術の適応を決定する上で有用な情報と考えられる. (3) 心室瘤群のうち心筋梗塞急性期以後に心不全を発症した群は, 非心不全群に比べ有意にSDが大きく, SD値が大きいほど心不全の危険性が増す. 以上より, 位相のSDは梗塞後心室瘤症例を非観血的に定量化するのに有用な一指標と考えられた.

Abstract[Summary] Quantitative evaluation of post-infarction left ventricular (LV) aneurysm was attempted by temporal Fourier analysis using multigated blood pool image (MGPI). Fourteen patients with post-infarction LV aneurysm, 19 AMI without LV aneurysm and 7 patients with angina pectoris were selected for MGPI. Data acquisition of MGPI were obtained at the LAO position. The value of LVEF obtained by MGPI (LVEF-RI) was compared with that culculated by cotrast LV angiogram (LVEF-cont.). Value of the standard deviation (SD) of the phase histogram was compared with % perimetric circumference of infarction (ΔL) and evaluated clinically. Good correlation was observed between LVEF-RI LVEF-cont. (r=0.82, p<0.01). The value of SD correlated well with ΔL of cases of post-infarction LV aneurysm. Lower limited value of SD was 21 degrees, and that was equivalent to the value of 35% in ΔL. The patients with congestive heart failure (CHF) in their clinical course had a higher value of SD and ΔL comparing with patients without CHF. It was concluded that the value of SD obtained by phase analysis provided a valuable information for non-invasive quantitative evaluation of post-infarction LV aneurysm.
