Abstract「要旨」原発性肝癌38症例について67Gaおよび減算シンチグラムの所見を選択的血管造影所見と対比した. 15例15個の細小肝癌のうち肝シンチグラム単独では5例のみを検出したに過ぎず, これに対して67Gaおよび減算シンチグラムにより合計11例の腫瘍を検出することができた. このように, 67Gaおよび減算シンチグラムは細小肝癌の検出能をたかめることを明らかにした. つぎに, 肝シンチグラムはび慢型および多発結節型の原発性肝癌の検出率は良いが, 孤立型の検出率が低い傾向がみられたのに対して, 減算シンチグラムはいずれの型についても検出率が良好であった. さらに, 原発性肝癌の腫瘍部の67Gaの集積度と血管分布の程度を比較し, 血管分布の豊富な腫瘍では67Gaの集積の強い場合が多いことを認めた.

Abstract[Summary] To assess the usefulness of the subtraction scintigraphy for the detection of small primary hepatic carcinomas and, also to elucidate the relationship between 67Ga-citrate uptake by the tumor, and both the gross form and the vascularity of the tumor, we have performed comparative studies among the findings obtained by 99mTc-colloid-, 67Ga-citrate-, the subtraction scintigraphy, and the selective angiography in the 38 cases of primary hepatic carcinomas. Among 15 tumor nodules with the diameter smaller than 5.0 cm, 5 nodules were detected by the colloid scintigraphy, 3 by the 67Ga-citrate scintigraphy and another 3 by the subtraction scintigraphy. The results indicate that the 67Ga-citrate and the subtraction scintigraphy is more preferable than the colloid scintigraphy for the detection of the primary hepatic carcinoma with small size. The gross form of the primary hepatic carcinoma was classified into diffuse (10 cases), multinodular (13 cases) and solitary type (15 cases) from the angiographic findings. The positive diagnosis was obtained by the subtraction scintigraphy in the whole of diffuse type (100%), in 12 of multinodular type (92%) and in 13 of solitary type (87%). The colloid scintigraphy showed a poor positive rate in solitary type (66.7%), compared with that in diffuse type (100%) and multinodular type (84.6%). These results suggested that the subtraction scintigraphy was more favorable than the colloid scintigraphy in the diagnosis of primary hepatic carcinomas of solitary type. The comparative study of the 67Ga-citrate uptake with the vascularity in the 31 cases of primary hepatic carcinomas with focal defects in the colloid scan, was performed. In all cases except one the tumors were hypervascular and in the patients with markedly hypervascular tumor the gallium uptake was prominent.
