Abstract | 「要旨」RI法により, 左室relaxationを評価するため, 78例 [健常者 (N) 17例, 狭心症 (AP) 22例, 心筋梗塞 (MI) 24例, HCM6例, CCM9例] を対象に, 心プールシンチグラムを施行した. 方法は, 99mTc-HSA20 mCi静注後, 平衡時にLAO-45°の方向より左室容量曲線を作成. そして, その一次微分が0の点より100msec後の容量増加量と拡張末期容量との比をearly filling volume ratio (%EFV) とした. また, 29例に心カテーテル検査を行い, Weissらの方法により時定数 (T) を求めた. %EFVとTとの相関は, Y=30+68/X, r=0.69, p<0.001と双曲線上に見られた. また, N (11.4±1.2) に比べ, %EFVはAP (6.1±1.5), MI (6.0±0.1), HCM (4.6±1.1), CCM (4.7±0.1) と有意 (p<0.001) に低下し, overlapは見られなかった. 今まで報告されたPFR, 1/3 FF, 1/3 FR-meanの指標は, Tとの相関はなく, また, 25%以上の病的心で, N群とoverlapした. 以上より, %EFVは, 左室relaxationおよび病的心の評価に有用な指標であった. |

Abstract | [Summary] To assess left ventricular (LV) relaxation, ECG gated cardiac blood pool study was performed in the LAO 45 view in 78 patients [17 normal (N), 22 angina pectoris (AP), 24 myocardial infarction (MI), 6 HCM, 9 CCM]. The ratio of increment of LV volume from minimal volume to 100 msec to the end-diastolic LV volume was expressed as early filling volume ratio (%EFV). The time constant (T) of LV pressure during isovolumic relaxation was measured in 29 patients by Weiss method. The correlation between % EFV and T was significant (Y=30+68/X. r=0.69, p<0.001). Compaired with N (11.4+-1.2), %EFV was significantly lower (p<0.001) in AP (6.1+-1.1), MI (6.0+-1.5), HCM (4.6+-1.1) and CCM (4.7+-1.1). The peak filling rate (PFR), the filling fraction during first third of diastole (1/3 FF) and mean first third filling rate (1/3 FR-mean), now used as the indices of LV diastolic performance, were not correlated with T. In addition, in PFR, 1/3 FF and 1/3 FR-mean, more than 25% of the patients with diseased heart had overlap with N, on the other hand, there was no overlap in %EFV between N and diseased heart. These results indicate that %EFV can reflect LV relaxation and is a sensitive index for evaluating LV early diastolic property. |
