Abstract「要旨」肝シンチグラフィ施行時に99mTc-Sn-コロイドをbolus注入し, コンピュータにより早期の血流動態を測定した. 肝・脾の時間放射能曲線から肝動脈相と門脈相の勾配の比を肝血流指数 (H. I.), 脾動脈相と肝動脈相の勾配の比を脾血流指数 (S. I.) とした. これらを各疾患群別にみると, 正常H. I. 0.65±0.19, S. I. 0. 31±0.18, 慢性肝炎H. I. 1.02±0.42, S. I. 0.34±0.23, 肝硬変症H. I. 2.65±2.01, S. I. 0.82±0.92, 原発性肝癌H. I. 1.90±0.73, S. I. 0.51±0.19, 転移性肝癌H. I. 0.92±0.11, S. I. 0.30±0.15であり, 肝硬変症, 原発性肝癌のH. I., S. I. は正常群に比し, 5%以下の危険率で有意に高値を示したが, 慢性肝炎, 転移性肝癌では両者共正常群と有意の差を認めなかった. 本法は肝シンチグラフィ施行時に簡便に併用可能であり, 被検者に新たな被曝を与えることなく, 肝および脾血行動態に関する情報が加わるので肝疾患診断に有用であると考えた.

Abstract[Summary] Quantitative assessment of hepatic and splenic blood flow detected by injecting Tc-99m-Sn colloid as a bolus was performed on 75 patients who were suspected of having liver disease. Using a computer, the hepatic and splenic time-activity curves were analyzed. Hepatic index was calculated as the ratio of arterial to portal blood flow. The peak time of the right kidney was corresponded to the junction of the arterial and portal phases of the hepatic curve. Splenic index was calculated as the ratio of splenic arterial to liver arterial blood flow. Hepatic and splenic indices had elevated in cases of liver cirrhosis and hepatoma than those of normal controls. There was no significant difference in the hepatic and splenic indices among chronic hepatitis, liver metastasis and normal subject. These noninvasive tests for the hepatic and splenic blood flow may be useful in writing a report of liver scintigram because of the added information of the liver.
