Abstract「I. はじめに」現在, ジメルカプトコハク酸テクネチウム(99mTc-DMSA, 以下Tc-DMSA)は腎スキャン剤として広く利用されているが, 腎臓のほかにも肝臓が描出されてしまうことが知られている. この原因についてはいくつかの報告がみられるが, ペーパークロマトグラフ法(PPC), 薄層クロマトグラフ法(TLC), ゲルカラムクロマトグラフ法(GC)などによるTc-DMSAの分離結果と動物での99mTcの体内分布により検討されているものがほとんどである. 一方, わが国では「放射性医薬品基準」によりTc-DMSAの純度試験法が定められており, 不純物の放射能は5%以下であることとされているが, 今回われわれはPPCを用いての分離結果とそれを注射して得られる腎シンチグラフィ特に肝の描出との関連について検討した.

Abstract99mTc-DMSA (Tc-DMSA) has been used as the renal imaging agent. However, the activity of technetium (99mTc) is often accumulated in the liver. The present research was performed on the association between the accumulation in the liver and Tc-DMSA solution which was given an injection to the patients. Tc-DMSA was separated to 2 complexes (complex I; C・I, complex II; C・II), when analyzed by the paperchromatography (SG-81 paper, Whatman) with pH 3-saline. The degrees of the liver accumulation were classified into 4 grades ((-)-(+++)). Therefore, the ratio of C・I/C・II was each 1.085+-0.402 on (-), 2.617+-0.888 on (+), 5.273+-1.791 on (++), 7.844+-3,178 on (+++), and determined the significant difference between (-) and (+). (p<0.001) On the other hand, the liver accumulation was not correlative to creatinine, BUN and urinary pH with patients. Thus, on analyzing Tc-DMSA by paperchromatography, it was found that at least 2 complexes 99mTc-labeled should be formulated in Tc-DMSA solution. The value of C・I correlated well with the degrees of the liver accumulation.
