Abstract | 「要旨」陳旧性心筋梗塞25例に, 運動負荷99mTc心プールイメージングを行い, 心予備能を評価するとともに, これに関与する因子の検討を行った. 運動時に左室駆出率(LVEF)は, 7例で上昇(Group 1), 12例で不変(Group 2), 6例で低下(Group 3)を示した. Group 3では, 他の2群に比し, 多枝疾患が有意に多く(Group 1 0/7, Group 2 1/12, Group 3 5/6, p<0.05)みとめられた. また, Group 2ではGroup 1に比し, 安静時LVEFが低値を示し(Group 1 53.9±7.4%, Group 2 42.7±11.2%, p<0.05), 両群の1枝疾患例における安静時201Tl心筋シンチグラムdefect scoreは, Group 2で高値を示した(Group1 28.6±13.6%, Group 2 44.3±7.3%, p<0.001). 以上より, 心筋梗塞の心予備能は一様でなく, 1)病変冠動脈数, 2)1枝疾患例では梗塞巣の大きさにより影響されると考えられた. |

Abstract | Twenty-five patients with old myocardial infarction (MI) were studied with gated cardiac blood pool imaging at rest and during two-stage exercise test to assess cardiac performance reserve, and the factors that influence cardiac performance reserve were determined. During exercise, left ventricular ejection fraction (LVEF) increased in seven patients (Group 1), remained the same in twelve patients (Group 2) and decreased in six patients (Group 3). The groups were comparable with regard to age, sex, localization of MI and left ventricular volume at rest. The prevalence of two- or three-vessel disease was higher in Group 3 (5 of 6) than other two groups (Group 1: 0 of 7, Group 2: 1 of 12). Group 2 had a lower LVEF at rest and higher myocardial scintigraphic defect score than Group 1 (LVEF: 42.7+-11.2% vs 53.9+-7.4%, p<0.05; defect score: 43.3+-8.1% vs 23.3+-11.3%, p<0.001). It is concluded that cardiac performance reserve is not uniform among individual patients with old MI, and the major determinant of cardiac performance reserve may be 1) the severity of coronary artery disease and 2) the extent of infarction in single vessel disease. |
