Abstract「I. はじめに」 近年 201Tl を用いた心筋シンチグラム (以下Tl-MPI) は, 虚血性心疾患の必須の診断法として繁用されている. 一方Tl-MPIにより心室形態の判読も可能であることから, 各種病的心における心室形態についての報告もみられる. 今回私どもは多方向Tl-MPIより, 左室長軸径および長軸方向左室平均曲率半径を算出し, 心形態を客観的かつ定量的に評価する方法を検討した. 「II. 対象ならびに方法」 対象は心疾患のない正常者9例 {心胸郭比 (以下CTR) 51.2±3.3%} を対照群とし, 右室負荷群として, 肺高血圧症 (以下PH) 5例 (CTR 52.4±8.1%), 僧帽弁狭窄症 (以下MS) 5例 (CTR 61.6±8.5%), 心房中隔欠損症 (以下ASD) 4例 (CTR 56.0±6.1%), 左室負荷群として僧帽弁逆流症 (以下MR) 5例 (CTR 56.2±9.6%), 大動脈弁逆流症 (以下AR) 9例 (CTR 53.3±7.4%) および弁膜症に由来しない心不全 (以下CHF) 8例 (CTR 67.2±10.2%) の計45名である.

Abstract[Summary] To evaluate morphological changes of left ventricle (LV) in various heart disease, 201Tl myocardial scintigraphy was performed in the 6 projections. The LV long axis and radius of curvuture was estimated from planar images. 51 patients [15 normal subjects, 14 patients with right ventricular (RV) overload (PH = 5, MS = 5, ASD = 4), 22 patients with LV overload (MR = 5, AR = 9, CHF = 8)] were evaluated. The scintigraphically estimated LV long axis was well correlated with the LV long axis estimated from cineangiography. The ratio of the short axis and the long axis was higher in ASD and CHF than normal subjects, whereas lower in AR. And the ratio of the radius of curvuture for the septal wall and the posterior wall was higher in RV overload groups than LV overload groups. The ratio of the radius of curvuture for the septal wall and LV long axis was also higher in RV overload groups. Thus the scintigraphical estimation of LV long axis and radius of curvuture was valuable to morphological evaluation of LV which was affected by pathological hemodynamic changes.
