Abstract「要旨」負荷心RIアンジオグラフィの実際の方法および, これによる虚血の検出について, 精度高く行えることを第1報で報告した. 本報では, 冠動脈造影を施行している心筋梗塞40例, 狭心症30例, 正常10例を対象として負荷前後の左室駆出分画の変動, 局所壁運動異常について検討した. 本法により, 虚血の検出の検出に加え, 心予備能の評価が可能であった. すなわち, 負荷時, LVEFが著明に低下する群は, 狭心症にて左主幹部病変, 多枝病変を有するもの, 心筋梗塞にて多枝病変を有するものであり, 後者では梗塞の部位みならず非梗塞部位の代償不全を把握でき, 虚血心における重症度判定に有用であった. ついで, 安静時心機能および99mTc-RBC肺/心臓比も重症度判定の指標として役立つことが示された. 以上の成績より, 負荷心RIアンジオグラフィは, 虚血の検出に加え, 心予備能の面から虚血の重症度判定が行うことができ, 虚血心の病態の把握に有用である.

Abstract[Summary] In order to detect coronary artery diseases (CAD) and to assess LV functional reserve, exercise radionuclide ventriculography (ERV) was performed in 80 patients who underwent coronary angiography for evaluation of chest pain syndrome. There were 30 patients with angina pectoris (AP), 40 patients with myocardial infarction (MI) and 10 normal cases. In CAD patients, changes of left ventricular ejection fraction (LVEF) from rest to exercise (ΔLVEF) were within less than 5%, while in normal cases, LVEF increased more than 5%. In 30 patients with AP and 10 normal cases, the sensitivity and specificity of ERV were 83%, 100%, respectively, and ERV had better sensitivity than stress ECG for the detection of CAD. In 11 cases of CAD, substantial decline of ΔLVEF (<=10%) indicated AP with multi-vessel or proximal LAD occulusion and MI with multivessel diseases. In the patients who had large MI, resting LVEF was lower and ΔLVEF did not significantly increased, however, lung/heart 99mTc-RBC ratio showed higher value in comparison with normal cases, which expressed the severity of pulmonary congestion. In conclusion, ERV was clinically useful not only for the detection of CAD, but also for the evaluation of LV functional reserve. Furthermore, it was demonstrated that high risk patients of CAD were identified by ERV.
