Abstract「要旨」心電図同期心プールシンチグラフィと同時に冠動脈造影, 左心室造影を施行した53症例を対象に, 一回拍出量 (SV), 駆出分画 (EF), フーリエ変換で求められる位相および振幅をパラメータとしてfunctional imageを作製した. 壁運動異常群では, 異常の程度が強くなるとEFは低下, 位相の標準偏差 (SD) は大きくなり, 両者に負の相関 (r= -0.73, p<0.001) を認めたが, 壁運動正常群ではEFが低値の症例でも位相のSDは正常範囲であった. 受信者動作特性 (ROC) 解析の結果, 位相およびEFの各イメージは, SVおよび振幅の各イメージより, 左心室壁異常運動の検出率は良好であったが有意差はなく, ROC曲線上有病正診率 (TPF) を0.70とすれば, 無病正診率 (TNF) は0.60-0.70であった. また位相のSD15°を正常および異常壁運動の閾値にとると, TPFは0.74, TNFは0.81であった. 左心室を3領域に分けて評価すると, hypokinesisの検出率は0.50以下であった. 壁運動異常の評価のためには, EF, 位相の各イメージの併用が適当と考えるが, 位相のSDなどの客観的指標を併用すれば, 視覚的判定に比して, 壁運動異常の検出率の向上が期待できる.

Abstract[Summary] Fifty-three subjects were studied by biplane contrast left ventriculography and gated blood pool scintigraphy. Stroke volume (SV), ejection fraction (EF) and phase and amplitude of fundamental wave of Fourier transform, were calculated to produce functional images. In patients with asynergy, regional EFs were low and standard deviations (SD) of phase were large and negative correlation between EFs and SDs of phase (r= -0.73, p<0.001) was recognized. But in patients without asynergy, there was no significant correlation between these two parameters. The detectabilities of asynergy by phase or EF images were better than SV or amplitude images, however, the difference of the detectabilities were not statistically significant. When true positive fractions (TPF) of four functional images were 0.70, true negative fractions (TNF) were 0.60-0.70 on ROC curves. When SD of 15° was used as a threshold of normokinesis and asynergy, TPF and TNF were raised to 0.74 and 0.81. TPF of the detection of hypokinetic region was less than 0.50. For the assessment of ventricular wall motion, the use of both EF and phase images were considered to be better and the detectability of asynergy by functional image will be improved by the use of objective indices such as SD of phase.
