Abstract「要旨」 123I-ortho-iodohippurate (123I-OIH) とコンピュータを接続したシンチレーションカメラを用いて, 連続腎シンチグラムを撮像し, さらに相対的分腎血流・機能の定量的評価を行い, 99mTc-DMSAおよび131I-Hippuranと比較検討を行った. 1) 123I-OIHと99mTc-DMSAの相対的分腎血流・機能指標は相関係数 r = 0.971 (p < 0.001), 回帰式 y = 1.026x-0.287であり, 両者の値は良く一致した. 131I-Hippuranと99mTc-DMSAでは相関係数 r = 0.968 (p < 0.001) と非常に良好であるが, 回帰式は y = 0.850x + 8.57であり, 123I-OIH群に比べて個々の症例でばらつきが認められた. 2) シンチレーションカメラの計数効率は123I-OIHは131I-Hippuranの2.44倍あり, 投与量当りでは6.1〜12.2倍の計数を得ることができた. また123I-OIHによるイメージの画質は良好で, ファントーム実験および症例においても, 形態の評価は131I-Hippuranに比べてはるかに優れていた.

Abstract[Summary] Quantitative measurement of relative separate renal perfusion and function, using 123I-ortho-iodohippurate (OIH) and a scintillation camera interfaced to a computer was performed and was compared with those of 99mTc-DMSA and 131I-Hippuran. 1) Relative renal perfusion and function ratios with 123I-OIH were well correlated with those of 99mTc-DMSA ; r = 0.971 (p < 0.001), y = 1.026x - 0.283. Correlation between the ratios with 131I-Hippuran and 99mTc-DMSA was also very good ; r = 0.968 (p < 0.001), y = 0.850x + 8.57. However differences of indivisual ratios were significantly higher in the group with 131I-Hippuran, which could be due to the low count of 131I-Hippuran. 2) The counting efficiency of 123I was 2.44 times that was obtained with 131I. Therefore count of 6.1 to 12.2 times was obtained with 123I-OIH per administered dose (0.5, 1.0 mCi) as compared to 131I-Hippuran (200 μCi) in serial renal scintigraphy. Quality of the images with 123I-OIH was also good in the phantom experiment as well as in patients with various renal diseases.
