Abstract | 「要旨」 負荷 Tl-201 心筋イメージング (以下 stress Tl-201 MPI) をより質的に評価するために, 独自のcircumferential profile法を開発した. 冠状動脈病変の確認された13例を対象に同法を用い, 空間的〔RD score : Crest-Cstress〕, 時間的〔WOI (K) : K = 1/T×1000〕解析を行い, 冠状動脈傷害度 (Pujadas score) と比較した. (1) Pujadas scoreとRD scoreとの間には有意の相関はないが, 前者が高値な程, 後者は低値を示す傾向が観察された. (2) 正常領域でのTl-201のwashout (WOI : 104±10) は速やかであるが, 虚血部位では負荷に伴うTl-201の取り込みの増加はみられずWOIは58±22と低値を示した. 画像上診断できない重症3枝障害例でのWOIは36±13と著しく低値を示した. (3) stress Tl-201 MPIの虚血巣の検出率は画像診断のみでは10/13 (77%) であるが, 時間的分析 (WOI) を加えることにより12/13 (92%) と向上し, 負荷心電図所見も加味すると100%であった. 画像診断のみならずTl-201活性の経時的動態の観察は心筋虚血を評価する上で有用と考えられた. |

Abstract | [Summary] To develop a more quantitative approach to Thallium (Tl)-201 myocardial imaging, spatial and temporal analysis was performed in 13 patients with coronary lesion over 75% in cineangiogram. Exercise Tl-201 MPI was performed by using treadmill exercise. Each image was smoothed by 9 point averaging algorithm and subtracted by the interpolated method. The images were displayed as circumferential profiles curves after devided into 16 redial segments. The magnitude of redistribution (RD score) was calculated by circumferential profile curves at the 10 minutes and 240 minutes after stress. The washout index (WOI) (K) was determind by the time-course of Tl-201 as K = 1/h×1000. These results were compared with the index (Pujadas score) determined by coronary angiography. No linear relationship was observed between Pujadas score and redistribution score. In some case with a high Pujadas score, the corresponding redistribution score showed a low value. Tl-201 activity (counts/pixel) in the normal zone significantly decreased, whereas no increase accom panied stress and slow washout of Tl-201 activity observed in the ischemic zone. In the typical ischemic zone nearequalization of Tl-201 activity was observed between normal and ischemic zone at 240 minutes. It is to be noted that WOI showed a low value (58+-22) in the ischemic zone compared with the value (104+-10) in the normal zone. In cases with tripple vessel disease and lision of left main truncus WOI showed significantly low value. In summary, quantitative criteria using these values have been suggested for the analysis and interpretation of the degree of severity in the ischemic heart disease. |
