Abstract「要旨」 In-vivo赤血球標識平衡時multi-gate法RN心アンギオを用い, カウント法より左室容積を求める方法を考案し, その有用性を検討した. 左室容積は, 131I-HSAで求めた循環血液量, 99mTc-04-全注入放射能, 平衡時左室ROI内の1心拍当りのテクネシウム放射能の3者より算出し, 絶対値(ml単位)で求めた. また, 種々の心容積を有する心肺ファントムを用い, 本法の基礎的検討も行った. ファントムを用いた, 本法による心容積値は, 実測値とよく一致した(r = 0.96). 各種心疾患19例における, 本法より求めた左室拡張終期容積は, 心エコー図法により求めた値とよく一致した(r = 0.86). また, 本法の再現性も良好であった. 本法を用いることにより, 平衡時multi-gate法RN心アンギオより左室容積を求めることが可能であり, 心機能のより詳細な評価が可能となると考えられた.

Abstract[Summary] Equilibrium multi-gated radionuclide(RN)angiography using in-vivo labeling of red blood cells by 99mTc were obtained from 19 patients with various cardiac diseases, and left ventricular volumes were determined by the method without geometric assumption. Blood pool images were obtained in modified 40° left anterior oblique projection. Preceding to RN angiography, circulating blood volume was determined with 131I-HSA. Then, the first transit data were obtained with the bolus injection of 99mTc, and total injected 99mTc activity was measured. 30 minutes after the injection of 99mTc, equilibrium studies were performed with multigated data acquisition. Each R-R interval was divided into 40 msec interval frames. Background corrected left ventricular 99mTc activity was estimated. Scintigraphic volumes were determined as follows : the ratio of LV 99mTc activity to total injected 99mTc activity was multiplied by circulating blood volume. Scintigraphic volumes were compared with echocardiographic volumes obtained just after completion of the RN study. In twelve patients, 99mTc red cell labeling efficiency was examined, and in nine patients, second RN study was performed one week later to assess the reproducibility of this method. A cardiac phantom was developed and serial volumes were studied. Excellent correlations were obtained between scintigraphic volumes and measured volumes in phantom study(r = 0.96, p < 0.01). Scintigraphic left ventricular end-diastolic volumes(LVEDV)were correlated well with echocardiographic LVEDV(r = 0.89, p < 0.01)/ Efficacy of Tc red cell labeling was 94% at 30 minutes after Tc injection, and remained greater than 95% up to 120 minutes. The reproducibility of LVEDV by RN method was good. In conclusion, radionuclide techniques may be utilized for the quantitation of left ventricular volumes.
