Abstract | 「要旨」 心拍同期心プール像のデータを位相解析(phaseanalysis)することにより左室内の位相分布ヒストグラム表示し, 臨床例47例について左室壁の時間的な収縮の不均等性(asynchrony)の定量化を試みた. ヒストグラムから算出された標準偏差(SD)は正常8例は全例10°以下となり, 心筋梗塞23例中21例(91%)が10°以上を示した. またSDは同時に算出された左室駆出率と有意な負の相関を呈し, これがasynchronyの程度と共に左心機能の良い指標になると考えられた. 左室造影でdyskinesisを呈した例はもちろん, hypokinesisを呈した例でも有意なSDの増大があり, 収縮の遅れの存在が示唆された. さらには, 心筋梗塞の一部にニトログリセリン投与によりasynchronyの著明な増大を示す例がみられた. phase analysisは, 局所壁運動の空間的変動(asynergy)のみでなく, 時間的な収縮不均等性(asynchrony)も評価でき, ヒストグラム表示により, それらを客観的かつ定量的に判定できると考えられた. |

Abstract | [Summary] Clinical evaluation of regional wall motion abnormality has been performed by submitting an equibrium gated brood pool study to a pixel by pixel temporal Fourier analysis. In an attempt to evaluate asynchronous contraction quantitatively, standard deviation(SD)of the phase histograms distributed within the left ventricle was calculated in 47 cases(normal subjects 8, myocardial infarction 23, angina pectoris l0 and hypertrophic cardiomyopathy 6). The SD in normal persons was less than 10°. However, in 21 of the 23 cases(91%)with myocardial infarction, it was more than 10°indicating the presence of asynchronous contraction. This abnormality was also suggested in one case with angina pectoris and one with hypertrophic cardiomyopathy. The SD was inversely correlated with left ventricular ejection fraction. Therefore, this index was considered to reflect left ventricular function as well as asynchrony. Delayed and asynchronous contraction was observed in cases showing not only dyskinesis but also hypokinesis in contrast ventriculography. Furthermore, asynchrony was markedly accentuated after sub-lingual nitroglycerine in 4 of the 14 cases with myocardial infarction. We conclude that temporal Fourier analysis can assess temporal as well as spatial abnormalities of regional wall motion quantitatively. |
