Abstract | 「要旨」 99mTc平板線源によるガンマ線transmission computed tomography(TCT)を用いることにより胸郭の輪郭像を得ることができた. 99mTc人巨大凝集アルブミン(MAA)による肺血流分布のsingle photon emission computed tomography(ECT)像を, 胸郭のTCT像とコンピューターを用いて重ね合わせ, 肺の辺縁での血流欠損を明瞭に描き出すことができた. TCTとECTを併用する方法(T-ECT法)は, 胸膜側よりの血流分布異常の診断上有用であると考えられた. 本法を肺線維症, 慢性気管支炎, 血管炎, 硅肺, サルコイドーシス, 心疾患, 肺炎, 卵巣腫瘍の12例に対して応用し, 6人の放射線科医による4方向からの通常肺シンチグラムの読影結果と比較した. 診断結果は, 全体の約8分の3に不一致を見た. 内訳は, 単純肺シンチグラムで欠損 「あり」 としたが, 本法では欠損が認められなかったものがそのうちの約3分の1, 逆に本法で欠損があったにもかかわらず, 単純肺シンチグラムからは 「なし」 としたものが残りの約3分の2に認められた. これらの診断については今後症例を重ねて行くことが必要と考えられた. |

Abstract | [Summary] A technique is described for transaxial imaging of the thorax during pulmonary perfusion study. The contours of the air-containing lungs were obtained by transmission computed tomography (TCT)of gamma rays from a 99mTc plane flood source. The transverse images of the rib cage were overlaid on those of the emission computed tomography(ECT)of 99mTc-macroaggregates of human serum albumin(MAA), which was injected for the perfusion study. Results of the ECT in conjunction with the ECT(T-ECT)were applied to evaluate the marginal hypoperfusions of the lungs in patients with pulmonary fibrosis, pneumonia and other cardio-pulmonary diseases. It permitted reliable estimates of the peripheral perfusion defects. Twelve patients were studied by this technique. The detection rates of the hypoperfusions on the pleural edges were compared between the T-ECT method and the planar 99mTc-MAA pulmonary scintigrams. Discrepancies were found in approximately three eighths in our cases. Further investigations were considered to be necessary to improve accuracies in the diagnoses. |
