Abstract[Summary] Quantitative assessment of aortic or mitral regurgitation and cardiac shunt in atrial septal defect is not so easy and usually based on clinical evaluation, supplemented by indirect techniques. For a more precise evaluation, a simultaneous measurement and a comparison of stroke volumes determined by cardiac catheterization and angiography are required. The stroke count ratio (SC ratio) for left and right ventricular end-diastolic, end-systolic count differences obtained from LAO multigated cardiac blood pool scintigram, was used to determine valvular regurgitant fraction or shunt ratio. Regurgitant fraction (RF) or shunt ratio (SR) was given by the formula: RF=1-1.48/SC ratio, SR=1-SC ratio/1.48. These were derived by assuming that an excess of stroke counts was due to regurgitant or shunt flow, and that the coefficient in counting ventricular radioactivity was 1.48, which was adopted mean value of SC ratio in normal subjects. Agreement was found between RF or SR obtained by this formula and by the invasive method. This technique may be useful for non-invasive and quantitative assessment of the severity of valvular regurgitation and cardiac shunt.
