Abstract「要旨」近年普及しつつある7ピンホールコリメータを用いた断層法(7P)と, 回転型ガンマカメラを用いたエミッションCT(SPECT)による202Tl心筋断層像の比較検討をファントム実験で行った. 撮像時間は7Pで10分, SPECTで22分であった. 両者とも水中で直径2cmの欠損を描出できたが, 解像力は7Pでは深部につれて低下し, また心尖部の欠損の評価ができない欠点があった. SPECTでは一旦データを収集すれば横断, 前額, 矢状断層像が得られ, いかなる位置の欠損もその最適断面で描出可能であった. また7Pでは欠損像が真に存在する面から前方および後方にも波及する傾向がみられ, 難点であった. 7Pは短時間の撮像が可能で, しかも従来のガンマカメラへの装着だけで簡便に断層像が得られる利点があるが, 今回の検討で明らかにされた種々の問題点を理解した上での利用が望まれる. 他方SPECTは, 予測された断層像を忠実に表わし, あらゆる角度からの評価が可能であり, 幅広い臨床応用が期待される.

Abstract[Summary] Single-photon emission computed tomography (SPECT) using a rotating gamma camera for thallium-201 myocardial imaging was evaluated employing a cardiac phantom in comparison with sevenpinhole tomography (7P). A 2 cm-diameter defect of the phantom placed in a water tank was visualized by both tomographic techniques. The SPECT providing transaxial, frontal, and sagittal sections, was able to detect the defect in any position in either of these tomographic sections, while 7P could not show the defect situated in the apical or deep regions of the phantom. The defect-to-normal wall count ratio (D/N ratio) in each image was determined in comparison with the expected activity for the reconstructed plane. The 7P provided slightly higher D/N ratio indicating lower image contrast. In addition, marked image-propagation forward and backward was observed, which implied longitudinal propagation into some reconstructed planes which did not contain it. On the other hand, the SPECT indicated high image-contrast without image propagation. Thus, the 7P being simple and requiring rather short time for data-collection and image-reconstruction, has severe quantitative limitations. On the other hand, the SPECT provided high spatial resolution, high image-contrast, and accurately reconstructed images. We conclude from our phantom study that the SPECT constitutes a sensitive and reliable method to demonstrate a perfusion defect of the myocardium in the evaluation of coronary artery disease.
