Abstract「要旨」正常者ならびに肝疾患を中心とした各種消化器疾患患者の血清ferritin値を測定し, その臨床的意義を検討した. 一般に良性疾患では, 肝炎および膵炎の急性期, 胆石症発作時, さらに肝硬変の非代償期に高値を示したが, 多くは症状の改善とともに下降した. 悪性疾患では, 肝癌, 膵癌, および食道癌で明らかな上昇がみられた. 肝細胞癌では, AFPとの相関は明らかではなく, AFPの低値例でferritinが高値を示す例もみられた. 肝硬変より発癌過程を追跡しえた例, および肝癌切除例ではAFPとferritinの解離がみられた. 他方, 転移性肝癌では, 大部分がferritinまたはCEAの高値を示し, 肝癌以外の消化器癌では, CEA 5.0 ng/ml以下の半数にferritinの上昇がみられた. 以上より, 血清ferritinの測定は, 単独では治療効果の判定ならびに発癌の早期診断の有用性に乏しく, AFP, CEAとの組み合わせにより, 特にこれらの低値群で診断的価値が認められた.

Abstract[Summary] Using two-site immunoradiometric assay, serum ferritin levels were determined in various types of digestive diseases, and compared with their AFP and CEA levels. Serum ferritin levels were found to be raised in malignant diseases, especially in cancer of the liver and the pancreas. However, there was no significant correlation between serum ferritin and AFP levels. In 60% of patients with hepatoma who had AFP levels less than 500 ng/ml, ferritin levels were more than 1000 ng/ml. The high levels of ferritin persisted even though AFP dropped postoperatively in patients with hepatoma. Two patients who were discovered hepatoma during their follow-up study of cirrhosis showed low levels of ferritin. Contrarily, the AFP increased progressively and significantly. In 50% of patients with other malignant diseases of digestive organs, whose CEA levels were below 5.0 ng/ml, increased ferritin levels were observed. From these results, we concluded that the assay of serum ferritin was useful for diagnosing of some malignant diseases of digestive organs, particularly in cases whose AFP and/or CEA was relatively low. However, it should be kept in mind that the ferritin alone was less valuable in detecting and monitoring these malignancies.
