Abstract「要旨」右心機能を容量面から評価することは, 右心室が左心室と比べて低圧系であり圧変化よりも容量変化の方がより大きな影響を与えるものと考えられるので重要である. しかし, 右心室容量測定の困難さゆえに, これまで左心機能ほど多く論じられていない. 本論文では, ガンマカメラと核医学情報処理装置を用いて右心室駆出率(RVEF)を測定し, 測定上の問題点を考察するとともに, 各種心疾患の右心機能について検討を行なった. 方法は99mTcO4-静注により得られる右前斜位のアンジオカルジオグラフィーのデータを解析しFirst pass法でのRVEFを求めると同時に, 対比検討を行なうため他の心機能指標, 左心室駆出率(LVEF). 心拍出量, 駆出量, 右心-左心時間を測定した. その結果は, 本法によるRVEFはシネアンジオ法によるRVEFと良い相関が得られ, NYHA心不全度と対比するとやや感度が劣るが, 僧帽弁膜症, 先天性心疾患, 肺性心などの右心負荷疾患および下壁心筋梗塞症などで著明に低下し, 重要な指標となり得る. また, LVEFと対比することで右心・左心機能の相互関係を知ることができ有用であると考えられた.

Abstract[Summary] Right ventricular function was assessed by quantitative first pass radionuclide angiocardiography (RIACG) in 73 patients with various cardiac diseases and in normal subjects. Right ventricular ejection fraction (RVEF) obtained with radionuclide were correlated well with those obtained with cineangiography (r=0.88 n=10). RVEF, 46.1 +- 5.1% in normals, was decreased in the patients with mitral valvular disease (29.0 +- 6.6% n=8), congenital heart disease (37.2 +- 7.2%, n=7), cor pulmonale (30.5 +- 3.7%, n=4), and old myocardial infarction (39.4 +- 9.9%, n=19). In the comparison with NYHA classification of CHF, however, RVEF was found to be inferior to other cardiac indices such as LVEF and Peak to Peak time to represent overall cardiac function. The ratio of RVEF to LVEF indicates the relation of right and left ventricular function, and remaind in normal range in all patients except for with old myocardial infarction and cor pulmonale.
