Abstract「要旨」2台のScintillation cameraで90度異なる2方向から同時に左心室血液プールのデータを採取し, Computer解析により左心室を3次元的にとらえ心室容積を算出する方法を考案し, その目的のためのソフトを開発して基礎的検討と臨床応用の検討を行なった. 本法による心室容積の算出法は, 心室が長軸を中心とする回転楕円球体であるとする理論に基づくものであるが, 従来の2次元的な解析法に比較して3次元的な解析が行なわれているために理論的根拠が強く信頼性が高い. 心室の3次元的解析にあたっては, 2方向からみた2次元イメーシが基となるため, 2次元イメージをいかに正確に表示するかが算出される心室容積の成績を左右する. そのための検討が行なわれ, 2次元イメージ表示条件が定められた. 平滑操作が加えられたイメージではほぼ70〜80%以下をCut offした条件で正しい大きさを示していたが, 心室容積が小さいほど高いCut offを必要とすることが知られた. 算出された心室容積の成績は, ±10%の誤差範囲で真容積を正確に表現していた. 本法は, 心室容積を絶対値として得られるため, この値から心駆出率, 1回拍出量, 分時拍出量, 容積曲線など心機能を示す各種指数が導かれ, 心機能診断への応用価値が高い. 2台カメラ同時記録解析法による左心室の3次元的解析法は, 非観血的に短時間のデータ採取で既報の形態学的診断, 心筋運動の診断をはじめ, 本稿の心室容積算出による心機能診断まで多面の情報が同時に得られ臨床応用に有効な方法である.

Abstract[Summary] The left ventricular blood pool data were recorded using two sets of scintillation cameras from two directions crossing each other in right angle simultaneously for computer analyses of the results, and thus the ventricular volume calculation software were developed, which were studied for basic experiments and applied clinically. This method of ventricular volume calculation is more reliable than other conventional methods since the three dimensional analysis are done in the process. As the calculation is done based on the two dimensional images of the two directions, it is necessary to express correctly the two dimensional images and for that purpose the adequate indication requirements were set. The required conditions depends on the size of the ventricle, but generally it was considered adequate to cut off the rest leaving 70-80%. The ventricular volume expressed by this system was in the accuracy of +- 10% of error. As the value of the ventricular volume obtained by this system is the absolute value, various indices showing cardiac functions such as ejection fraction, stroke volume, minute output, volume curve, etc. could be derived, so that its diagnostic applicability is quite high. The three dimensional analysis of the left ventricle in the method of synchronous dual camera recording and analysis is a useful clinical system for simultaneous diagnoses of morphological and functional aspects of a ventricle and of the myocardial movement as it enables sampling of the ventricular data in a short time with a noneinvasive technic.
