Abstract | 「要旨」われわれは小児の腹部腫瘤の診断に腎のscintigraphyを多用してきた. 今回過去10年間に行なった58例の腎scintigram(static imagingのみ)を検討し, 各疾患における特徴的所見を示した. 昭和49年までは203Hg chlormerodrin, 昭和50年からは, 99mTcDMSAを用いた. 99mTcDMSAおよびscinticameraの使用により, 診断能の明らかな向上が示されている. Wilms腫瘍は, 欠損像, 腎の位置異常が特徴であり, 特に両側性Wilms腫瘍の治療経過の観察に有効であった. 神経芽細胞腫は, 腎上極の特有な外圧迫による平低化および腎の下方への位置異常が特有であった. 水腎症では腎皮質が大きく広がった像が特有である. 後腹膜奇形腫(cystic type), 偏側性多嚢腎では腎像が欠損している時, 腎床部はむしろlow uptake areaとなっている. |

Abstract | [Summary] We have very often used kidney scintigraphy for the differential diagnosis of abdominal tumors in pediatric ages. This time we analysed 58 scintigrams of static imaging in the past 10 years' experience, and discussed on the characteristic findings of various disorders. 203Hg-chlormerodrin was used up to 1974, and thereafter 99MTc DMSA was applied. Remarkable improvement in the diagnostic property was obtained with the application of 99mTc DMSA under scinticamera technique. In Wilms' tumor, defect image and kidney displacement were characteristic. Renal scintigraphy was very effective in the evaluation of treatment course of bilateral Wilms' tumor. In neuroblastoma, a unique flattening of the upper pole of kidney due to the external compression from above was characteristic. Kidney tended to displace straightly downwards. In hydronephrosis, renal parechyma seemed to be grossly streched in the scintigram. When the renal image was absent in the case of cystic type of retroperitoneal teratoma and unilateral multicystic kidney, the area of renal bed had low uptake characteristically. |
