Abstract | 「要旨」著者らは, 腎内局所におけるRI動態を画像として表現できる腎functional imageを作成, すでに報告してきたが, 今回, 尿路閉塞性疾患43例にて, 腎functional imageを適用, その臨床的有用性について検討した. 方法は, 131I-hippuranによる経時的腎シンチグラムデータを用いた. 各症例にて, レノグラム, 腎シンチグラムと比較するとともに, 水腎症を岡, otnesらの腎盂造影の分類に基づき4群に大別した. この結果, 腎functional imageは, 各群にて特徴的な画像で表現されることがわかった. すなわち, 水腎症の程度に応じ, Tmax, Compartment数の増加, Down Slopeの画像では欠損像として表現され, 残存皮質機能と明瞭に区別された. 特に, 腎実質障害の程度がUp Slopeの画像にて勾配の低下として表現される. このように, 本処理を用いることにより, 腎の機能, 形態の両面から水腎症の病態の把握に有用である. 本法は水腎症における診断, 治療のみならず経過観察, 手術適応の決定に役立つと考えられる. |

Abstract | [Summary] Functional image of the kidney, the parametric map of the regional renal function was developed using Iodine-131 hippuran sequential scintigraphic data. An Anger camera and on-line minicomputer system was employed for the data processing. A dynamic curve in each element of the digitized image was extracted and the pertinent parameters (Cmax, Tmax, Up Slope, Down Slope, Fixed Time Slope, Compartments) were calculated to display as parametric maps on a CRT. In this study, 43 cases of obstructive uropathy were studied and the functional images were compared with the findings of renogram, scintigram, intravenous pyelogram, selective angiogram and laboratory data. Forty-three cases of obstructive uropathy were divided into four groups of hydronephrotic change by intravenous pyelogram, i.e., 11 cases of dilated pelvis, 8 of atrophy of papillae, 16 of large globar calyces, 8 of huge hydronephrosis. In generally, in the case of hydronephrosis, significant delay of Tmax and increase of number of Compartments were demonstrated in image at the hydronephrotic portion. These portions were expressed as defect area in the image of Down Slope, in which the remaining cortex with normal excretory function was clearly discriminated. And image of Up Slope reflected renal parenchymal function corresponding to the degree of impairment. In conclusion, according to the degree of hydronephrosis, each parametric map showed the charcteristic image which reflect clinical significance in obstructive uropathies. Moreover, the functional images in the obstructive uropathy were applied to the indication of operation and management of the hydronephrotic patients. |
