Abstract | 「I. はじめに」結節性甲状腺腫, とりわけ甲状腺癌の非侵襲的な診断法として核医学検査のはたす役割は大きい. 現在広く行なわれている123I, 131I, 99mTcO4-による甲状腺シンチグラムでのcold noduleの質的診断は難しく, 甲状腺癌を陽性描画する試みがなされてきたが臨床的に満足できるものは少ないようである. 最近, 塩化タリウム - 201Tl (以下201TlClと記載) が甲状腺癌に集積し, 有用な陽性描画医薬品として注目されてきた. しかし癌以外の結節性甲状腺腫にも集積することが多い. われわれは癌と非癌とを鑑別する一方法として, この201TlClを用い, 静注後10分のいわゆるearly scintigraphyに加え, 静注後2時間経過したdelayed scintigraphyを行ない, 集積度の変化を観察した. その結果, 癌と非癌との間に興味ある差を認めたので報告する. |

Abstract | [Summary] Thallous chrolide-201Tl (201TlCl) tends to readily concentrate in thyroid nodules. This fact prompted us to apply the 201Tl scintigraphy to differentiate malignant thyroid nodules from benign ones. Thus scintigraphic study has been carried out for such cases that have shown cold nodules by 131I and high uptake in early scan by 201Tl (vide infra). The early thyroid scintigraphies performed within 10 minutes after intravenous administration of 201TlCl were followed by delayed ones 2 hours later for 9 cases. The scintigrams of 5 cases of the thyroid carcinoma (papillary tubular adenocarcinoma) all exhibited high abnormal activities in the early image corresponding to the cold nodules by 131I, and the activities remained clearly detectable in the delayed scintigrams. For 4 benign cases (2 chronic thyroiditis and 2 adenomatous goiter), on the other hand, the abnormal activity disappeared in the delayed image although these cases also showed strong accumulations of 201TlCl in the early study corresponding to the nodules. It was found that the abnormal activity in malignant thyroid nodules remains in the delayed scintigram while the activity in benign nodules vanishes. Thus it is quite useful to perform a delayed scintigraphy in addition to early one using 201TlCl in differentiating malignant thyroid nodules from benign ones. |
