Abstract「要旨」: 骨シンチ用の塩化第一錫を加えたピロリン酸 (Sn-PYP) を用い99mTcによる赤血球のin vivo標識法について検討した. 最適な標識方法は, Sn-PYPを30μg/kg (SnCl2・2H2Oとして) 静注し30分後に99mTcO4-を静注する方法であった. 標識率は平均96%であり, 赤血球と99mTcの結合は5時間以上維持されていた. また, 使用する99mTcの量, 性状による標識率の変化はほとんど認められなかった. この方法は従来のin vitro標識法に比し標識手技が簡単で同程度の標識率が得られ, 推奨されると考える. 臨床的にも心血管系のBlood pool scanに使用し, ほぼ満足できる結果を得た.

Abstract[Summary] Recently, in vivo labeling method of red blood cells with 99mTc-pertechnetate has been reported for blood pool scaning. In this study, optimal yield of in vivo labeling of 99mTc-RBCs and its clinical utilization were investigated. The labeling of 99mTc-RBCs were performed by separate intravenous administration of stannous pyrophosphate (Sn-PYP) and 99mTc-pertechnetate. In order to determine the optimal doses of Sn-PYP 24-80μg SnCl2/kg of Sn-PYP was injected into ten volanteers. Then 2 ml of blood were withdrown for the invitro labeling of 99mTcO4- at the time of 15, 30, 60, and 120 minutes after Sn-PYP injection. Counting ratio of total blood to RBCs was measured by gamma-well counter. Forty-five patients were studied by taking the images of the heart and the great vessels using gamma-camera in clinical studies. Also, 2 ml of blood samples were withdrown to determine the labeling efficiency of 99mTc-RBCs. In these investigations, following conclusions were obtained. They are as follows : 1) The optimal doses of Sn-PYP were 30 μg SnCl2/kg. And no significant increase of RBC labeling ratio was obtained in the amount of stannous ion. 2) The optimal time interval of two injections of Sn-PYP and 99mTcO4- was 30 minutes. 3) Neither significant changes were noted in the amount of carrier 99mTc (Containing in the 99mTc-pertechnetate) nor the amount of 99mTc-pertechnetate at the range of 5-20 μCi per 1 ml blood. 4) The average yiled of 99mTc-RBCs was 96%. And separation of 99mTc and RBC was not observed up to 5 hours. 5) In vivo labeling of 99mTc-RBCs is an easier and more efficient than that of in vitro labeling. Clinically, high quality blood images of the heart and the great vessels as well as the peripheral vessels were obtained.
