Abstract | 「要旨」肝胆道シンチグラフィー薬剤として新しく開発された99mTc-HIDAについて131I-Rose Bengalとの血液停滞率, 尿中排泄率, 131I-BSPとのイメージを比較した. さらに99mTc-HIDAによるregional hepatogramの解析についても報告した. 99mTc-HIDAは, 131I-RBに比較して, 血液クリアランスは速いが, 尿中排泄率が高かった. 131I-BSPとの比較では, 99mTc-HIDAは肝細胞から胆管へのRI移行が速やかで, 胆管・胆のうが非常によく示された. 特に, 胆のう炎兼胆石症では92% (12/13) に胆のう描出がなく, 胆のう疾患と胆のう描出欠損の関連が示唆された. 99mTc-HIDAのregional hepatogramおよび各胆管領域のdynamic curve解析は今後, 肝胆道疾患の病態生理の解析に新たな情報を提供することが示された. 99mTc-HIDAは新しい肝胆道シンチグラフィー薬剤として充分臨床的価値を有する薬剤であると結論することができる. |

Abstract | [Summary] 36 patients with a hepatobiliary disease have been studied with a new hepatobiliary scintigraphic radiopharmaceutical, 99mTc-HIDA. 99mTc-HIDA has been available as a commercial freezed dry kit containing 10mg of dimethyl HIDA and 0.25mg of stannous choloride produced by Frosst Co. in Montreal, Canada. In this article were discussed blood clearance and excretion ratio in urine by 2 hours postinjection in comporison between 99mTc-HIDA and 131I-R.B. and qualitative discrepancy in sequential images of 99mTc-HIDA compared to 131I-bromsulphthalein. 99mTc-HIDA was proved to have preferable characteristics for excellent visualization of biliary tree system due to fast blood clearance, prompt hepatocyte transit and high concentration excreated in biliary tree system, especially in patients with hepatobiliary disease below 10mg% of total serum bilirubin which seems to be critical in clinical diagnostic value. A modified subtraction method associating regional hepatograms processed with a computer (Tosbac-40, DAP 5000 Nsystem) was also done for proper evaluation of pricise biliary tree abnormality which was not obtained alone with sequential analog images. As a result of this study, 99mTc-HIDA is likely to be fairly a good radiopharmaceutical of clinical value in patients without hyperbilirubinemia over about 10mg% of total bilirubin even if it has a relatively high excretion ratio in urine. |
