Abstract | 「要旨」骨シンチグラフィを骨隣接部に発生した軟部腫瘍18例に行い, その軟部腫瘍がX-P上明らかに骨破壊をもたらしている症例については, 骨原性腫瘍と同様のシンチグラム所見を呈するか否か, また骨破壊が明瞭でない症例については, 初期の骨破壊の検出の可能性について検討した. 前者(8例)では, 悪性浸潤性, 良性圧迫性骨破壊を問わず, 一般に骨原性腫瘍に比し, 放射能の異常集積程度が低いという結果が得られた. ことに8例中2例では, 異常集積像が全く認められなかった. また後者(10例)からは, いわゆる"minimal bone destruction"の検出の可能性は否定的であった. 以上の知見から, 骨原性腫瘍と軟部腫瘍による2次的骨破壊とでは, シンチグラム所見に相違があると推定された. |

Abstract | [Summary]Soft part tumors adjacent to bone not infrequently produce destruction of the bone, invasive or erosive in fashion. The utility of 99mTc-diphosphonate imaging was evaluated in 18 cases with various soft part tumors, both malignant and benign in nature, including 8 cases with and 10 cases without evidence of bone destruction on roentgenogram. Although six cases in the series of 8 cases with bone destruction showed slight to moderate increase in radioactive accumulation in the site of the tumor including the bone lesion, the focal deposit of radioactivity was not so marked in degree as usually observed in the primary bone tumors. No increase of radioactive accumulation in bone lesion was found in two cases with definite bone destruction (erosive destruction of cervical vertebrae by neurinoma and invasive destruction of pelvic bone by metastatic rhabdomyosarcoma). In ten cases without evidence of bone involvement, including a case of rhabdomyosarcoma which was surgically suspected to be the minimal infiltration to the femur, no radioactive deposit was observed in the bone adjacent to the tumor. Conclusively, bone scintigraphy may be useful in differenciation of secondary bone involvement of soft part tumor from the primary bone lesions. There seems to be no possibility to scintigraphically detect presence of the minimal bone destruction produced by soft part tumor. |
