Abstract体の病変部をシンチグラム上に描出する場合には, 用いた放射性医薬品 (RI) がそこにとり込まれて集積像として現される方が欠損像となる場合よりも発見しやすいことは言うまでもない. 従って癌診断の分野では以前から腫瘍親和性RIの開発が試みられ, まだ腫瘍だけに集積するものはないとは言うものの, すでにかなりの成果があがっている. ところで, 骨のシンチグラムに関しては, 以前から用いられていた85Srは幸いなことに骨の病変部に集まるためシンチグラムは大変有益な情報を提供し, 骨のX線写真よりもしばしば早い時期に病変を指摘してくれていた. しかし, これもやはり骨折や炎症などの非腫瘍性の病巣にも集積することは, さきに述べたいわゆる腫瘍親和性RIと同様であった. この85Srは半減期が65日と長いために投与量もせいぜい50〜60μCi程度におさえられ, この程度の量ではスキャンスピードの速い全身スキャナを用いるには不向きであった.

Abstract[Summary]Since Subramanian reported the usefulness of 99mTc-tripolyphosphate as a bone-seeking agent in 1971, 99mTc-phosphate compounds have been accepted widely in this field. The authors report here the results of clinical studies of whole body scintigrams taken with the compounds of four different types, which are 99mTc-polyphosphate suppled by A Company, 99mTc-pyrophosphate by B Company, and 99mTc-pyrophosphate of electrolysis method and 99mTc-diphosphonate by C Company. As far as these four compounds were concerned 99mTc-diphosphonate showed the most favorable result with a constantly good scintigram quality. The remaining three compounds were found also favorable for clinical application. Although these compounds are useful, there are problems due to undesirable accumulations in a whole body scintigram, which we should be aware of. These are some accumulations in nasal base, anterior neck, and inferior angle and upper margin of scapula, asymmetrical accumulations in joints, spotty accumulations in sternum, accumulation in urinary bladder, different accumulation in long bones upon the ages, occasional liver uptake, accumulations in ribs due to previous thoracic surgery, and also accumulation due to extravasation. Besides these undesirable accumulations, rather low energy of 99mTc causes different pictures upon the anterior and the posterior views, and previously irradiated area shows a decreased uptake of these compounds. The usefulness of whole body scintigrams in the detection of metastatic bone tumors was also discussed. There was a definite superiority with this technique over the conventional X-ray study. Especially in thoracic vertebrae, pelvic bone, and ribs, far many diseased areas were detected on the scintigrams than the areas which the physicians noticed or suspected the metastases. As a conclusion, although there are some problems on the scintigrams, it can be said that 99mTc-phosphate compounds are very useful in detection of metastatic bone tumors, especially with the help of whole body scanner.
