Abstract「I はじめに」99mTcは核種自身の有する物理的優秀性 (物理的半減期は6時間, シンチグラフィに最適な140KeVの単一γ線エネルギー放出) と, 実際使用するに便利な99Mo-99mTcカウが容易に得られることにより, 万能シンチグラフィ用核種として期待されている. 肝シンチグラフィ用としても従来より99mTc-サルファコロイドが用いられ, 肝内限局性病変の検出, 脾描画, 患者への被曝量軽減など, 198Au-コロイドに比してその優秀性が認められていることは周知の事実である. しかし反面, 調製時に約20分と多少時間を費やすこと, またコロイドの粒子サイズの均一性など種々の問題が指摘されてきた. 近年, 99mTc-スズコロイドや99mTc-フチン酸) が相ついで報告され極めて短時間内に調製可能となった. そこで今回, シンチスキャナーを用いて99mTc-スズコロイドと99mTc-フチン酸による肝シンチグラフィを施行し, 従来の198Au-コロイドと99mTc-サルファコロイドとの比較検討を試みた.

Abstract[Summary]Liver scintigraphy with 99mTc-sn-colloid or 99mTc-phytate was done in over 200 cases. Each of these two 99mTc-compounds could be more rapidly and easily prepared than 99mTc-Sulfur Colloid. In the present study, the comparison between each of these two 99mTc-compounds and 99mTc-Sulfur Colloid or 198Au-colloid was done clinically, and the following results were obtained. 1. Hepatic accumulation coefficient (KL) : Hepatic accumulation curves were recorded in the same patient, using 99mTc-Sulfur Colloid,99mTc-sn-colloid and 99mTc-phytate. The result of these three hepatic accumulation coefficients (KL) calculated from the curves showed that a) the value of 99mTc-sn-colloid was about the same as that of 99mTc-sulfur colloid, and b) the value of 99mTc-phytate was the smallest among these three compounds. 2. Degree of extrahepatic activity (especially, splenic visualization degree) : It is very use fulfor the evaluation of diffuse hepatic disease to decide the abnormal splenic visualization degree. For the purpose, the comparison of splenic visualization degree was done on anterior view between 198Au-colloid and each of these two compounds. The following findings were considered to be abnormal : a) A splenic activity was equal to or exceeded the hapatic activity in 99mTc-sn-colloid scan. b) The splenic activity was obviously found in 99mTc-phytatescan, since in such cases the splenic activity was usually found in 198Au-colloid scan. The order of splenic visualization degree among these three 99mTc-compounds was considered as follows : 99mTc-Sulfur-Colloid > 99mTc-sn-colloid > 99mTc-phytate. Both of 99mTc-sn-colloid and 99mTc-phytate were considered to be superior to 198Au-colloid and 99mTc-sulfur colloid, because of sharper scintigram with less radiation dose than 198Au-colloid and more rapid and easier preparation than 99mTc-Sulfur Colloid, and these two 99mTc-compounds could be fully appicable into liver scintigraphy for the evaluation of diffuse as well as focal hepatic diseases. Moreover, although the particle size of these two compounds could not be evaluated accurately, it was inferred that a) the particle size of 99mTc-sn-colloid was about the same as or a little smaller than that of 99mTc-Sulfur Colloid, and b) the particle size of 99mTc-phytate in the body was about the same as that of 198Au-colloid.
