Abstract | 「はじめに」各種の副腎疾患において副腎病変特に副腎腫瘍の局在の診断は, 単なる血中ホルモン測定によっては困難で, 各種のレントゲン検査による診断法が行われて来た. しかし, これらの方法では必ずしも確実に副腎病変を把握することは出来ない. 近年, 各種の臓器に特異的に集積する放射性化合物を用いて, その臓器の形態的異常を診断しようとする試みがなされ, ほとんどすべての臓器についてこれが可能となった. 副腎についても, 副腎疾患が種々の全身的な代謝異常を来たすので, 副腎に特異的に集積する放射性化合物の開発が必要視されていた. 1971年, ミシガン大学のBlairらはコレステロールがステロイド生成の原料となる所から, 131Iをlabelしたコレステロールが副腎のスキャン剤として適当であるとの見解に基づき, 安定な放射性化合物として131I-19-コレステロールを開発し, 副腎病変の診断に応用した. |

Abstract | 131I-19-iodocholesterol was used in visualizing the adrenal glands of 36 patients including 13 patients with primary aldosteronism, 10 with Cushing's syndrome, one with pheochromocytoma, 2with 17α-hydroxylase deficiency and 9 with normally functioning adrenal tissue. Six to 9 days after the intravenous administration of the dose, the patient's adrenal glands were imaged with 5 inch crystal rectilinear photoscanner. Normally functioning adrenal glands were readily visualized after administration of 131I-19-iodocholesterol. In 13 patients with primary aldosteronism, the photoscan gave a positive indication of the side of the lesion in a gland containing an adenoma, with a normal radioactivity concentration in the opposite gland. Seven patients with Cushing's syndrome due to ACTH excess showed adrenal glands approximately equal in size and radioactivity concentration. An adrenal adenoma in three patients with Cushing's syndrome definitely visualized on scan, with no uptake in the opposite gland. In a patient with a large pheochromocytoma, the involved gland was not imaged, but"normal"adrenal image of the normal contralateral glands was shown. In two patients with 17α-hydroxylase deficiency, both demonstrated bilaterally equal sized adrenal glands. The biological half-life of the 131I-19-iodocholesterol was 2.2〜52.60 days, and by calculation, the effective half-life was 1.77〜1.98 days. |
