Abstract | 「はじめに」最近, 遊離型Thyroxine濃度(以下Free T4とする. )の指標として, Effective Thyroxine Ratio(ETR)を測定するRes-O-Mat ETR Testが開発された. ETRは血中T4結合Globulin(TBG)のT4結合能の異常に左右されず. よく甲状腺機能を反映し, 甲状腺機能のScreening Testとして有用と考えられた. しかし乍ら, ETR値のみではTBG異常症は見落される. そこで著者らはRes-O-Mat ETR TestでETR値と共に血中T4濃度が測定出来る様にその操作法を改良した. かくして, TBG異常症の発見と, 甲状腺機能の診断をより確実にする事が可能と考えられたので, ここに報告する. 「検査対象」検査対象は健康人15例, 未治療甲状腺機能亢進症患者15例, 未治療機能低下症患者10例, 正常妊婦6例, 甲状腺機能亢進症を合併した妊婦1例, 並びにTBG減少症4例計51例である. |

Abstract | (1)The ETR test was modified to determine the serum thyroxine concentration and the ETR value simultaneously. First of all, the determinations were made of serum thyroxine concentrations by the same procedures as the Res-O-Mat T4 test, using the ETR test. Thereafter, the strip was added again to the incubation mixture and serum micropipet was also added to the mixture. After another one hour incubation, ETR values were obtained. (2)Both serum thyroxine concentrations and ETR values obtained by the modified ETR test agreed closely with those by the original method. The patients with diminished TBG had low thyroxine concentrations and normal ETR values, whereas those with elevated TBG had elevated thyroxine concentrations and normal ETR values. Thus, it was possible to find out patients with abnormal TBG by the modified method. (3)Both ETR values and serum thyroxine concentrations were diminished in patients with untreated hypothyroidism, whereas these values were elevated in those with untreated thyrotoxicosis. Moreover, borderline cases of thyrotoxicosis had high-normal or slightly elevated ETR values and their thyroxine concentrations in serum were slightly elevated. In conclusion, the modified ETR test, by which serum thyroxine concentrations and ETR values were determined simultaneously, was more useful in vitro thyroid test than the original method. |
