Abstract「緒言」甲状腺機能の程度を正確に知るためには血中thyroxineおよびtriiodothyronine量を直接測定することが必要であるが, これらを直接測定することは現在もなおはん雑であり, 必ずしも容易でない. したがって方法的に簡便で, しかも正確に甲状腺の病態を反映する血中甲状腺ホルモンの測定法を開発することが従来とも急務とされてきた. われわれはこの意味においてMallincrodt社で開発されたRes-O-Mat T4 kitを用いて, 血中thyroxine量 (T4) を測定する機会をえたので, 本法による測定値と各種甲状腺病態, および既存の甲状腺機能検査法との関連について検討を加えた.

Abstract[Summasy] The Res-O-Mat T4 test, which fundamentally concerned with the competitive protein-binding analysis, was studied as a link in a series of clinical evaluation studies of the thyroid function tests. Patients with hyperthyroidism or hyothyroidism, as well as normal control subjects, patients with simple goiter, Hashimoto's thyroiditis or thyroid carcinoma and pregnancy or other miscellaneous dysproteinemias, were subjected to the study, and the results were compared with the results of other thyroid function tests, i, e., basal metabolic rate (BMR) , protein-bound iodine (PBI) , 131I-triiodothyronine resin sponge uptake rate (T3 RSU) , and serum thyroxine level as estimated with the Tetrasorb-125 kit. In addition, the results were compared with serum levels of β-glucuronidase activity, the association of which with functional state of the the thyroid has been reported by our instltute. 1) Influences of either oral or intravenous administrations of radiological contrast media containing organic iodine and the influences of changes in incubation period were studied, and it was found that these did not affect the results of the Res-O-Mat T4 test. On the other hand, longterm freeziug and thawing of serum samples caused to decrease the Res-O-Mat T4 value. 2) The mean value and standard deviation in each group are listed below : euthyroid 8.6+-2.9γ% hyperthroid 20.9+-7.1γ% hypothyroid 6.9+-1.2γ% non-toxic goiter 9.3+-3.2γ% Hashimoto's disease 9.7+-4.8γ% thyroid carcinoma 11.7+-3.4γ% Pregnancy 9.8+-1.3γ% cirrhosis of the liver & nephrotic syndrom 6.6+-1.9γ% 3) The results were compared with other thyroid function tests and the following coefficients of correlation were obtained : BMR 34 cases r=0.59 (p<0.001) PBI 29 cases r=0.73 (p<0.001) β-glucuronidase activity 34 cases r=0.70 (p<0.001) T3 RSU 43 cases r=0.41 (p<0.01) Tetrasorb-125 kit 35 cases r=0.75 (p<0.001) 4) Changes of value of the Res-O-Mat T4 test in 7 untreated hyperthyroid patients were observed during the administration of anti-thyroid drugs. The values were antecedently decreased in the improved cases of symptoms and reflected the effet of treatment. 5) From these reults, it is concluded that this test is sufficiently simple and reliable in estimating the functional states of thyroid.
