Abstract要約: 1) Tetrasorb Kitによる血中甲状腺ホルモンの測定法について, その原理ならびに測定法について紹介し, われわれの成績を述べた. 2) T4値は正常者119例, 6.4〜15.6μg/100ml平均10.9μg/100ml, 甲状腺機能亢進症82列, 15.1〜26.0以上μg/100ml, 平均24.2μg/100ml, 甲状腺機能低下症42例, 0.8〜4.9μg/100ml, 平均2.8μg/100ml, 瀰漫性甲状腺腫37例, 4.8〜15.5μg/100ml, 平均10μg/100ml, 結節性甲状腺腫37例, 7.0〜14.3μg/100ml, 平均11.3μg/100ml, 悪性甲状腺腫6例, 8.7〜12.3μg/100ml, 平均10.3μg/100ml, 亜急性甲状腺炎4例, 14.2〜260以上μg/100ml, 平均腺炎13列, 21.9μg/100ml, 慢性甲状2.7〜10.9μg/100ml, 平均7.2μg/100ml, 131I治療後治癒した甲状腺機能亢進進症60例, 4.7〜15.6μg/100ml, 平均9.9μg/100ml, 妊婦7例, 12.6〜17.0μg/100ml, 平均14.3μg/100mlであった. 3) T4値の正常範囲をわれわれは以上のdataより5.5〜15.0μg/100mlと仮に定めたが, 正常者119例中117例(98%)はこの範囲にあり, 甲状腺機能亢進症82例は全例(100%)が15μg/100ml以上であり, 甲状腺機能低下症42例も全例5.5μg/100ml以上であり, 正常者, 甲状腺機能亢進症, 甲状腺機能低下症の間の成績の重なり合いが著るしく少なかった. 4) T4値に0.653を乗じたT4I値とPBIとの相関はγ=0.95であった. 5) T4値とTriosorb値との相関は, γ=0.82で全体としてはよく相関したが, 正常者では両者の値の間にはかなりのバラツミがみられた. 6) Evaporationに使用するガスなど三, 三の基礎的問題について検討した.

Abstract[Summary] We studied the procedure and principle of determination on serum thyroxine by competetive protein binding analysis using Tetrasorb Kit and reported our clinical results in involving more than 400 patients in various thyroid status. Results of corrected values of serum thyroxine were as follows. 6.4〜15.6μg/100ml (mean value 10.9μg/100ml) for 119 euthyroid, 15.1〜more than 26.0μg/100ml (mean value 24.2μg/100ml) for 82 cases with hyper thyroidism, 0.8〜4.9μg/100ml (mean value) 2.8μg/100ml for 42 cases with hypothyroidism, 4.8〜15.5μg/100ml (mean value 10.0μg/100ml) for 37 cases of mon toxic diffuse goiter, 7.0〜14.3μg/100ml (mean value 11.3μg/100ml) for 37 cases of non-toxic nodular goiter, 8.7〜12.3μg/100ml (mean value 10.3μg/100ml) for 6 cases of malignant goiter, 14.2〜more than 26.0μg/100ml (mean valve 21.9μg/100ml) for 4 cases of subacute thyroiditis, 2.7〜10.9μg/100ml for 13 cases of chronic thyroiditis, 4.7〜15.6μg/100ml (mean value 9.9μg/100ml) for 60 cases of cured hyperthyroidism with 131I and 12.6〜17.0μg/100ml (mean valve 14.3μg/100ml) for 7 cases of pregnant women From these results we arrived at the conclusion that from 5.5μg/100ml to 15μg/100ml was an adequate range for euthyroid. Thyroxine value in all cases of hyperthyroidism (83 cases) and hypothyroidism (42 cases) were more than 15μg/100ml or less than 5.5μg/100ml respectivelly and in 117 cases out of 119 euthyroid cases were between 5.5μg/100ml and 15.0μg/100ml When PBI values were plotted against thyroxine iodine values, the correlation coefficient r=0.95 was obtained. When 131I-T3 Resin sponge uptake values were compared with serum thyroxine values, the correlation coefficient γ=0.82 was obtained but the correlation between two tests for euthyroid range was not so good. In various thyroid diseases, values of serum thyroyine using Tetrasorb Kit demonstrated high diagnostic accuracy, less overlapping, and good reproducibility than other thyroid function tests. Furthermore, this test was not affetced by iodine or mercury contamination and the course of thyroid drug and redioiodine therapy could be followed.
