Abstract | 131I-MAAを選択的に動脈内に注入したのちスキャニングを行なう方法をViamonte et al., がangioscanography (以下ASG)と名付けて以来, 主に悪性腫瘍についての報告がある. 血管造影で腫瘍血管が明らかでない症例で, 陽性スキャンがえられるときにASGの意義はもっとも大きい. 腎動脈においてもASGは悪性腫瘍の診断に有利であるが, 良性疾患においてもかなり有力な情報がえられることがある. われわれは最近, 腎動脈においてASGを行なっているが, 本法を他の放射線診断法に比較した場合の有用性について検討してみた. 「方法」一般に行なわれている選択的腎動脈造影を行なうが, 下副腎動脈, capsular artery等からも腫瘍血管が生ずることもあるので, 造影剤および131I-MAAが十分にこれらの血管へも注入されるようカテーテルの先端より1cm以内に2個の側孔をあけた. |

Abstract | [Summary] Angioscanography named by Viamonte et al. was performed in 26 patients with known renal or pararenal disease. We evaluated in cnmparison with other radiological diagnostic procedures. In renal malignancies, positive scannings were obtained according to tumor vessels on the angiograms. In a case demonstrating only a faint suspicious vessels on the angiograms, marked positive scannings were obtained. But in a necrotic malignancy, same as on the renoscintigrams, defect was demonstrated. In cystic kidneys, owing to associated impaired renal function and gas in the colon, other radiological procedures sometimes could not provide us exact localization of the cysts. Being not influenced by impaired renal function, we could obtain exact cystic condition on the angioscanograms. Although angioscanography was consistent with renoscintigraphy in acute hydronephrosis, angioscanography combined with renoscintigraphy is one of the most valuable diagnostic procedures in atrophic stage. In general, speaking of the localization of the lesion, angioscanogrms are valuable diagnostic procedure in the lesion of the cortex of the kidney with impaired renal function, suprarenal and pararenal mass. |
